QueensCare Health Centers / QueensCare Care Coordinator-LVN in Los Angeles, California

Job Description

We are seeking a Care Coordinator-LVN to work to improve the quality of patient care through effective and efficient utilization of ambulatory disease management protocols in collaboration with the patient care team. This individual will be expected to travel to different clinic locations as needed. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care through the integration and functions of case management, utilization review and patient self-care management.

Job requirements:

  • California LVN license
  • Current CPR
  • High school diploma or equivalent
  • Fluent in English/Spanish
  • California Drivers License and active automobile insurance
  • Community health experience preferred
